(i) मैं एतद् द्वारा विधिवत घोषणा करता हूँ /करती हूँ कि उपर्युक्त दी गयी सभी सूचनाएं सत्य हैं। तथ्यों को छिपाया या गलत तरीके से प्रस्तुत नहीं किया गया है। मैंने विवरणिका में प्रवेश प्रक्रिया सम्बन्धी सभी सूचनाएं तथा तथ्यों को अच्छी प्रकार से पड़ लिया है तथा मैं इनका अनुपालन करूंगा/करूंगी। यदि दी गई प्रविष्ठियों तथा संलग्नक, प्रमाण-पत्रों में कोई त्रुटि/विसंगति पाई जाती है तो मेरा प्रवेश निरस्त कर दिया जाए। ऐसी स्थिति में शुल्क वापसी के लिए किसी भी प्रकार का दावा नहीं करूंगा/करूंगी।
(ii) मैं यह घोषणा करता हूँ/करती हूँ कि मैंने आवश्यक सभी मूल प्रमाण-पत्रों की स्कैन कॉपी अपलोड की है तथा प्रमाण-पत्र सत्यापन के समय अगर इसमें कोई त्रुटि/विसंगति पाई जाती है तो इसकी पूर्ण जिम्मेदारी मेरी होगी ।
(iii) विश्वविद्यालय द्वारा निर्मित नियमावली के तहत कुलपति/कुलसचिव अथवा अन्य अधिकृत व्यक्ति द्वारा आवश्यकता पडने पर नियमानुसार की गई किसी भी प्रकार की अनुशासनिक कार्यवाही मुझे स्वीकार्य होगी।
(i) I hereby duly declare that all the information given above is true. Facts have not been concealed or misrepresented. I have thoroughly read all the information and facts related to the admission process in the brochure and I will comply with them. If any error/discrepancy is found in the entries made and the attached certificates, my entry may be cancelled. In such a situation, I will not make any claim for refund of fees.
(ii) I declare that I have uploaded the scanned copies of all the original certificates required and if any error/discrepancy is found at the time of certificate verification, it will be my full responsibility.
(iii) Under the rules made by the University, any kind of disciplinary action taken as per the rules by the Vice-Chancellor/Registrar or any other authorized person, if necessary, will be acceptable to me.
A) General Instructions
1. You are responsible for accuracy of information and filling in the form in the desired manner. You should ensure that you fulfill the admission criteria as prescribed by the University as on the last date for submission of Application form for Admission to B.A. Yoga (Hons) & M.A. Yoga Programme.
2. Forms submitted by ineligible candidates shall be summarily rejected and no
refund shall be made to such applicants.
3. A fee of Rs.500/- for Registration shall be payable by online payment mode
using debit card/credit card/Internet banking.
4.The instructions regarding conduct of Entrance Test or otherwise shall be
uploaded on the University’s website after the last date of online registration
5.For the Entrance Examination, the instructions for the same shall be uploaded on the University's website.
6.Applicants are instructed to visit University’s website regularly for the
latest and detailed information regarding admission to B.A. Yoga (Hons) & M.A. Yoga Programme/Entrance
Test.

Applicant may proceed as per the admission procedure and guide lines issued by
UOU.
