(i) मैं एतद् द्वारा विधिवत घोषणा करता हूँ/करती हूँ कि मेरे द्वारा प्रस्तुत
समस्त सूचनाएं सत्य हैं। तथ्यों को छिपाया या गलत तरीके से प्रस्तुत नहीं किया गया
है। मैंने विवरणिका में प्रवेश प्रक्रिया सम्बन्धी सभी सूचनाएं तथा तथ्य पढ लिए हैं
तथा मैं इनका अनुपालन करूंगा/करूंगी। यदि की गई प्रविष्ठियों तथा संलग्नक प्रमाण-पत्रों
में कोई त्रुटि/विसंगति पाई जाती है तो मेरा प्रवेश निरस्त कर दिया जाए। ऐसी स्थिति
में मैं शुल्क वापसी के लिए किसी भी प्रकार का दावा नहीं करूंगा/करूंगी।
(ii) मैं यह घोषणा करता हूँ/करती हूँ कि मैंने सभी स्व-प्रमाणित प्रपत्र इस
आवेदन पत्र के साथ संलग्न कर दिए हैं।
(iii) विश्वविद्यालय द्वारा निर्मित नियमावली के तहत कुलपति/कुलसचिव अथवा अन्य
अधिकृत व्यक्ति द्वारा आवश्यकता पडने पर नियमानुसार की गई किसी भी प्रकार की अनुशासनिक
कार्यवाही मुझे स्वी कार्य होगी।
(iv) उत्तराखण्ड मुक्त विश्वविद्यालय की समस्त परीक्षाएं केवल
उत्तराखण्ड राज्य में ही सम्पन्न होंगी।
(v) बी0 ए0 में विषय संयोजन विवरणिका के अनुसार करें। गलत संयोजन
लेने पर प्रवेश मान्य नहीं होगा ।

नामांकन संख्या आवंटन के उपरांत ही द्वितीय पाठ्यक्रम में प्रवेश ले सकेंगे (केवल

महत्वपूर्ण निर्देश
1.प्रवेश आवेदन पत्र में केवल स्वयं का ही मोबाइल फोन नम्बजर
दें, यही मोबाइल फोन नम्ब्र आपका Registered Mobile Number(RMN) होगा। एक मोबाइल फोन
नम्बेर से केवल एक ही आवेदक को प्रवेश दिया जायेगा।
2.प्रवेश आवेदन पत्र में केवल स्वयं का ही ई-मेल आईडी दें, यही
ई-मेल आईडी आपकी Registered Mail Id मानी जायेगी। एक ई-मेल आईडी से केवल एक ही आवेदक
को प्रवेश दिया जायेगा।
3.प्रवेश आवेदन पत्र में केवल स्वएयं का ही आधार नम्बर/सरकारी पहचान
पत्र नम्बर दें।
(i) “I hereby declare that the particulars stated in this application form
are true to the best of my knowledge and belief. I have read the prospectus and
the admission/eligibility requirements as laid down by the University and I shall
abide by them. In the event of suppression or distortion of any fact in my application.
My admission will stand cancelled at any point of time. In case my admission is
cancelled by the University due to any reason. I shall have no claim for refund
of fee paid by me.
(ii) I hereby declare that I have attached all the required self-attested
documents with this form.
(iii) Further I declare that I shall submit myself to the disciplinary jurisdiction
of the Coordinator of the study centre. The Vice Chancellor and Other authorities
of the University who may be vested with the power to exercise discipline under
the Act. The statutes. The Ordinances framed by the University. In this regard these
authorities have the right to take necessary disciplinary action.
(iv) All examinations of Uttarakhand Open University will be conducted
only in Uttarakhand State.
(v) In BA Programme please choose subject combination according
to University’s brochure. The admission will not be valid incase you enter the wrong
combination.
ANTI-DRUG DECLARATION FOR THE STUDENT
1. I am aware that the possession, use, sale and distribution of alcohol/tobacco/any psychoactive substances are wrong and harmful.
2. I shall refrain from using, being under the influence of, possessing ,furnishing,
Distributing, selling or conspiring to sell possess, or being in the chain of sale
Or distributing of alcohol/tobacco/any psychoactive substances within the
Premises of the institute/university of during any sponsored activities by the
Institute/university.
3. I shall report to the authorities of the institution any irregular behavior that I
Observe in relation to the possession, use, sale and distribution of
Alcohol/tobacco/any psychoactive substances which may have occurred at the
Institution or during any activities conducted by any students or institution.
4. I shall support and actively participate in any substance use prevention education
Programmes which may be organized by the institution/government which would
Enable me to be a better student and citizen of India.
5. I shall co-operate with the authorities of the institution and other relevant
Authorities in their investigation of any substance-related incident of which I may
Have information, and to prevent the possession, use sale and distribution of
any psychoactive substances in or around my institution.

enrollment number in first Programme (Only for online applicants)
Important Instructions
1.Mention your own mobile number only in your admission form;

one applicant shall be given admission in one mobile number.
2.Mention your own Mail Id only in your admission form; this mail-Id
shall be considered your Registered Mail Id; only one applicant shall be given admission
in one Mail Id.
3.Mention your own Aadhaar number / Government Identity Card number
only in your admission form.
