(i) मैं एतद्वादारा विधिवत घोषणा करता हॅू/करती हॅू कि उपर्युक्त दी गयी सभी सूचनाएं सत्य हैं। तथ्यों को छिपाया या गलत तरीके से प्रस्तुत नहीं किया गया है। मैंने विवरणिका में प्रवेश प्रक्रिया सम्बन्धी सभी सूचनाएं तथा तथ्य पढ लिये हैं तथा मैं इनका अनुपालन करूंगा/करूंगी। यदि की गई प्रविष्ठियों तथा संलग्नसक प्रमाण-पत्रों में कोई त्रुटि/विसंगति पाई जाती है तो मेरा प्रवेश निरस्त कर दिया जाए। ऐसी स्थिति में मैं शुल्क वापसी के लिए किसी भी प्रकार का दावा नहीं करूंगा/करूंगी।

(ii) मैं यह घोषणा करता हॅू/करती हॅू कि मैंने सभी स्वर-प्रमाणित प्रपत्र इस आवेदन पत्र के साथ संलग्न‍ कर दिये हैं।

(iii) विश्वविद्यालय द्वारा निर्मित नियमावली के तहत कुलपति/कुलसचिव अथवा अन्यव अधिकृत व्यक्ति द्वारा आवश्यकता पडने पर नियमानुसार की गई किसी भी प्रकार की अनुशासनिक कार्यवाही मुझे स्वीकार्य होगी।

(iv) उत्‍तराखण्‍ड मुक्‍त विश्‍वविद्यालय की समस्‍त परीक्षाएं केवल उत्‍तराखण्‍ड राज्‍य में ही सम्‍पन्‍न होंगी।



(i) “I hereby declare that the particulars stated in this application form are true to the best of my knowledge and belief. I have read the prospectus and the admission/eligibility requirements as laid down by the University and I shall abide by them. In the event of suppression or distortion of any fact in my application. My admission will stand cancelled at any point of time. In case my admission is cancelled by the University due to any reason. I shall have no claim for refund of fee paid by me.

(ii) I hereby declare that I have attached all the required self-attested documents with this form.

(iii) Further I declare that I shall submit myself to the disciplinary jurisdiction of the Coordinator of the study centre. The Vice Chancellor and Other authorities of the University who may be vested with the power to exercise discipline under the Act. The statutes. The Ordinances framed by the University. In this regard these authorities have the right to take necessary disciplinary action.

(iv) All examinations of Uttarakhand Open University will be conducted only in Uttarakhand State.
